Getting Started
Scheduling your first appointment is simple and can be done by phone or online. Depending on your insurance plan, a referral may be required prior to your visit. Our team is available to assist you with scheduling and any questions you may have.
What to Bring
To ensure a productive visit, please bring all necessary information and documentation.
This includes:
- Government-issued ID
- Insurance card
- List of current medications (including supplements)
- Referral letter from your primary care physician (if applicable)
- Prior medical records, imaging reports and CDs, or test results (if available)
- Copays

Preparing ahead of time helps maximize the value of your appointment. We recommend completing any required forms in advance and taking note of your symptoms, including when they started, how often they occur, and any triggers or patterns you have noticed.
What to Expect During Your First Visit
Your first visit is focused on understanding your symptoms, medical history, and overall health. This allows us to develop a clear and personalized plan for evaluation and treatment.
Arrival and Check-In Process
You will receive a text message on your phone to complete the check-in process online within 48 hours prior to scheduled appointment. Completing the check-in process online will make your visit run smoothly. You will be able to sign consent forms, complete medical questionnaires, upload a photo of your ID and insurance card and pay any amount due for the visit. When you arrive at the office you will just click I arrived on your phone and you will be checked in.
Please arrive 15 minutes early to allow time for check-in. Our staff will verify your information, collect any required documents, copay, and ensure all forms are completed before your appointment begins.

If your appointment is scheduled as a virtual visit, you will receive instructions in advance. Please ensure you have a stable internet connection and access to a device with video capability. Joining from a quiet, well-lit environment helps ensure a smooth consultation.
