Scheduling your first appointment is simple and can be done by phone or online. Depending on your insurance plan, a referral may be required prior to your visit. Our team is available to assist you with scheduling and any questions you may have.

  • List of current medications (including supplements)
  • Referral letter from your primary care physician (if applicable)
  • Prior medical records, imaging reports and CDs, or test results (if available)

What to Expect During Your First Visit

Arrival and Check-In Process

You will receive a text message on your phone to complete the check-in process online within 48 hours prior to scheduled appointment. Completing the check-in process online will make your visit run smoothly. You will be able to sign consent forms, complete medical questionnaires, upload a photo of your ID and insurance card and pay any amount due for the visit. When you arrive at the office you will just click I arrived on your phone and you will be checked in.

Please arrive 15 minutes early to allow time for check-in. Our staff will verify your information, collect any required documents, copay, and ensure all forms are completed before your appointment begins.

If your appointment is scheduled as a virtual visit, you will receive instructions in advance. Please ensure you have a stable internet connection and access to a device with video capability. Joining from a quiet, well-lit environment helps ensure a smooth consultation.
